The Health Collective's Community Health Worker (CHW) Program equips local residents with the skills and certifications needed to address health disparities and promote health equity in Hartford. Through our free, four-month program, participants receive comprehensive training, including CPR and healthcare navigation, as well as hands on experience through paid mentorships.
This initiative empowers community members to become health advocates and leaders in their neighborhoods, improving both health outcomes and access in the Greater Hartford area.
Key Features of the Program
Certification
Each year, 10-15 participants receive industry-recognized certifications, including CPR, and are trained to navigate complex healthcare systems.
Mentorship
Participants gain valuable, hands-on experience working alongside healthcare professionals through paid mentorships, enhancing their practical knowledge.
Engagement
The program includes workshops focused on civic engagement, health equity, and social justice, fostering a deep understanding of the social determinants of health.

What is a Community Health Worker?
A community health worker is a frontline public health worker who is a trusted member of, and has a close understanding of, the community being served. This trusting relationship enables the worker to serve as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
A community health worker also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
